Program Forms
There are two ways for a person to become a donor in the Anatomical Donation Program. The first is through signing up for the Anatomical Donation Registry. This is intended for living individuals who would like to donate the bodies upon their death. To enter into the Anatomical Donation Registry, you will complete the “Registration for Anatomical Donation” form, which signifies intent to donate your body upon passing. This can be rescinded at any time.
Individuals can also enter into the program after their passing through donation of their body by family members or an authorized representative. In this case, family members will complete the “Consent for Anatomical Donation” form upon the death of the donor.
